The Next Step in Integrated Care

As a patient-centered medical home, Doctors Care strives to offer our patients not only quality care, but also convenience by providing a variety of integrated care services in a “one stop shop” environment. The leadership team at Doctors Care regularly discusses new and innovative ways to optimize and expand our services, and then work to find partnerships and funding opportunities to make those innovations possible.

arrowcircle3smallFor example, a year ago, we formed a partnership with Arapahoe Douglas Mental Health Network that resulted in a behavioral health counselor being added to our provider roster. Our patients may now come in for a well visit and see a counselor in the same day – all within the Doctors Care Clinic. We recently added a team of patient navigators to assist our patients with socio-economic barriers such as transportation struggles, which can make it difficult to get to medical appointments. At Doctors Care, navigators are available to meet with patients and help find solutions for whatever individual challenges that may be experiencing.

What next?  Oral health! Studies show that taking care of dental hygiene needs at a young age pays back with a gleaming, healthy smile, and it may keep your heart healthy too!  Because oral health is integral to whole health, Doctors Care has added a staff Dental Hygienist to meet those needs. Our patients now have the opportunity to have their teeth examined and cleaned right in the convenience of their medical home.

Doctors Care is pleased to announce it has received a $5,000 grant from Douglas County Community Foundation. Douglas County’s generous support will guide the work of Doctors Care to provide compassionate, quality health care to the community. Funding will also support the provision of resources, connections, and education so that patients can build a foundation for a healthier future. In addition to providing comprehensive medical, behavioral, and dental health care, Doctors Care understands the importance of addressing the social determinants of health when treating south metro Denver’s most vulnerable populations. Heath education and resource navigation are vital components of its integrative approach to care and help individuals find a path towards long-term health and wellbeing.  “We are so grateful to the Douglas County Community Foundation for helping us continue providing high-quality care to individuals and families,” said Bebe Kleinman, CEO, Doctors Care.
